Poly-L-lactic acid (PLLA) biostimulator fillers (e.g. Sculptra) are powder fillers that are TGA and FDA-approved for aesthetic treatment of volume depletion (great on temples and pre-auricular areas), correction of shallow to deep nasolabial fold contour and other facial wrinkles.  It is approved for the treatment of lipoatrophy (facial fat loss) associated with the disease.


Origin
PLLA is a “biodegradable, biocompatible, immunologically inert substance that has been used in dissolvable sutures and dental implants.  This product consists of microspheres of PLLA that are 40-60 microns in diameter that initially expand tissue after injection by purely mechanical means but gradually biodegrade and stimulate collagen formation as a foreign body.’

Mechanism of action

PLLA does not technically lift or fill the face.  Similar to calcium hydroxylapatite (brand: Raddiesse), PLLA acts as scaffolding for endogenous collagen protein to form, restoring volume with natural results.

Efficacy
The effectiveness of PLLA on disease-associated lipoatrophy (facial fat loss) is supported by randomized trials and uncontrolled studies that have demonstrated improvement in patient appearance and cutaneous thickness.  

Multiple other studies have supported the effective use of PLLA for nasolabial folds, acne scars, facial asymmetry and other cosmetic treatments. The duration of effect exceeds that of hyaluronic acid and collagen fillers.

Procedure
Lift Aesthetics Sydney follows the recommended protocol of reconstitution with sterile water to ensure adequate treatment of the areas of concern.  

Immediately post-procedure, swelling and transient tissue oedema will show an appearance of full correction, but this swelling settles in 1-2 weeks.  

Review and top-up will be every 3-4 weeks until the treatment plan is completed (guide of using 1 vial of Sculptra every decade).  

Biostimulator results progressively improve over time, hence Dr Liz usually undercorrects.  Best results are noted after 3-6 months.

Results of improved skin quality, improved skin laxity, volume and skin thickness will last more than 2 years with proper correction.  

Adverse effects

Avoid adverse effects with proper dilution of the product, avoidance of poor injection technique (superficial injections and overcorrection), and manual tissue massage immediately after injection are necessary.

Potential adverse effects: haematoma, bruising, oedema, discomfort, inflammation and erythema.
Rare asymptomatic, subcutaneous papules weeks to months after treatment and delayed granulomatous formation will be avoided by patient selection (not injected on patients prone to inflammatory conditions, or have autoimmune conditions).  Nodules typically resolve spontaneously without intervention or by subcision, excision or intralesional steroid therapy.

Do not inject on patients with a history of hypersensitivity to product components and abnormal scarring such as keloid or hypertrophic scar formation.

This is a popular treatment due to its natural result on the face. You do not look filled at all.
With adequate treatment (1 vial per decade), effects last more than 2 years.
